Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. For well over a decade, Concordia Visiting Nurses has provided high-quality skilled nursing, palliative care, and a variety of therapy services to our patients. Concordia Visiting Nurses is seeking Physical Therapists for full-time or part-time positions which will cover parts of our service areas in and around Route 28. We are also hiring Physical Therapists in Butler, Apollo, Parker and Monroeville allowing you to work close to your home.

What You'll Do as Physical Therapist :

• Conduct comprehensive evaluations and develop individualized treatment plans that empower patients to reach their maximum potential.
• Implement evidence-based therapeutic interventions, including manual therapy, therapeutic exercises, and modalities.
• Monitor patient progress, adjust treatment plans as needed, and ensure timely and thorough documentation.
• Educate patients and their families on treatment options, rehabilitation techniques, and preventative measures.
• Collaborate with physicians and other healthcare professionals to ensure a holistic approach to patient care.
• Help create a welcoming and supportive clinic environment where patients feel comfortable and motivated

Are you a Physical Therapist that meets the requirements below?

• A graduate of an accredited Physical Therapy program with a valid state license.
