LMSW Social Worker

We seek a highly skilled and experienced Licensed Masters of Social Worker (LMSW) with at least 2 years of acute care or medical/surgical experience. The LMSW will provide social services support in an acute care setting, collaborating closely with interdisciplinary teams to address patient needs. Strong communication and adaptability to varying schedules, including some weekend and holiday coverage, are essential for success in this role. Responsibilities:

• Provide social services assessments and interventions for patients in acute care/medical-surgical settings.
• Collaborate with healthcare teams to develop and implement patient-centered care plans.
• Coordinate discharge planning and community resource referrals.
• Support patients and families through counseling and crisis intervention.
• Maintain accurate documentation of social work activities and patient progress.
• Participate in interdisciplinary team meetings and case conferences.

• Licensed Masters of Social Worker (LMSW) with an active compact license.
• Minimum of 2 years experience in acute care or medical/surgical social work.
• Strong communication and interpersonal skills.
• Ability to work independently and collaboratively in a fast-paced environment.
• Eligibility for travel position with required separation from previous Providence-affiliated employment.

• Experience working in hospital or acute care settings.
• Familiarity with discharge planning and community resources.

• Must be willing to cover some weekends as needed.
• Expectation to work 1 of 3 major holidays (Thanksgiving, Christmas, or New Year's).
• All Swedish facilities honor active compact licenses.
• Travel position requiring prior separation from Providence health system for a minimum of 3 months.

• Standard schedule is 5 days a week, 8-hour shifts, primarily Monday through Friday.
• Flexibility required to cover occasional weekends and holidays.
