Job Description Northeast Delta Human Services Authority is seeking a qualified individual to serve as a Social Service Counselor 3 to provide services through the SSI/SSDI Outreach, Access, and Recovery (SOAR) program, a national model designed to assist individuals who are experiencing or at risk of homelessness and have a serious mental illness, medical impairment, and/or co-occurring substance use disorder. The goal is to reduce and/or eliminate homelessness for individuals with serious mental illness, medical impairments, and/or substance use disorder who may qualify for SSI/SSDI and are experiencing homelessness. Qualifications Five years of social services experience. Six years of full‑time experience in any field plus two years of social services experience. A bachelor's degree plus two years of social services experience. An advanced degree in a social sciences field plus one year of social services experience. Experience Substitution: Every 30 semester hours earned from an accredited college or university will be credited as one year of experience towards the six years of full‑time work experience in any field. The maximum substitution allowed is 120 semester hours, which substitutes for a maximum of four years of experience in any field. Necessary Special Requirement: Positions that provide addictive disorder counseling or prevention services will require certification or eligibility for certification issued by the Addictive Disorder Regulatory Authority (ADRA), or its successor. Job Duties Collaborate with referral sources and community partners to identify applicants through team meetings, outreach, and referrals. Conduct interviews with individuals to gather information for SSI/SSDI applications. Collect medical records and supporting documentation to complete SSI/SSDI applications. Prepare SOAR Medical Summary Reports for individual applications. Accompany individuals to appointments at the Social Security Administration. Coordinate visits to medical doctors, psychiatrists, and other specialists to obtain evidence for applications. Provide and coordinate case management services with partners to support individuals. Meet the target population where and when needed. Travel for client meetings, training, stakeholder collaboration, and off‑site coordination with personnel/management. Assist the team with administrative tasks as needed. Participate in the annual Point‑in‑Time Count. Cross‑train in other job duties (such as SOAR Outreach Worker) to provide backup support to co‑workers. Adhere to all program policies and procedures. Position‑Specific Details Appointment Type: Job Appointment Location: Monroe Integrated Care Clinic (MICC) Compensation: The NEDHSA is a designated approved National Health Service Corps (NHSC) site. Clinicians who work at NHSC‑approved sites are eligible to apply to the NHSC Loan Repayment Program to pay off student loans. 2‑Year Full‑Time Clinical Practice: NHSC will pay up to $50,000* for an initial 2 years of full‑time clinical practice to clinicians. * 2 years of full‑time clinical practice is defined as no less than 40 hours per week, for a minimum of 45 weeks per year.
