Community Habilitation Specialist - Per Diem Job Category : OUTREACH Requisition Number : COMMU001314 Posted: March 6, 2025 Part-Time Location Troy, NY 12180, USA Description The Community Habilitation Specialist is responsible for coordinating and assisting individuals and families to achieve specific goals by providing respite, residential habilitation services, skill building, and parent assistance services. This is a Per Diem position ($20/hr). Primary Duties & Essential Functions: Maintain knowledge of community-based resources, including age-appropriate recreational activities. Provide direct care and services to individuals and families. Assist individuals with goal-oriented activities based on the agreed-upon service plan. Attend and participate in supervision, staff development opportunities, and agency/community committees. Communicate and interact with individuals and families appropriately and effectively. Explain and promote the Sanctuary Model to prospective employees, volunteers, and the community. Create a personal schedule based on individuals served utilizing appropriate time management techniques. Complete written service summaries as required. Complete assigned paperwork, including intake assessments and discharge summaries in a timely manner. Other tasks and responsibilities may be assigned as business needs require. Required Education, Knowledge and Skills: Valid NYS Driver’s License with a clean driving record. High School diploma or GED required. Ability to manage time and workload effectively. Ability to communicate effectively, orally and in writing. An equivalent combination of education, experience, knowledge, skills, and abilities.
