About the Role:

North Point Senior Homecare in Virginia Beach, VA is looking for a compassionate and dedicated PRN Licensed Practical Nurse (LPN) to join our growing team. Responsibilities:

Provide skilled nursing care to senior clients in a home care setting on a PRN (as-needed) basis
Administer medications, injections, and treatments as prescribed by the physician
Monitor and document patient health status, vital signs, and changes in condition
Develop and follow individualized patient care plans in coordination with the supervising RN
Communicate effectively with patients, families, and the healthcare team regarding care updates
Perform wound care, catheter care, and other clinical procedures as directed
Educate patients and family members on health management and care techniques

Requirements:

Active Licensed Practical Nurse (LPN) license in the state of Virginia
Previous home health, geriatric, or senior care experience preferred
Current CPR/BLS certification
Reliable transportation and valid driver's license for home visits
Strong clinical assessment and documentation skills
Compassionate, patient, and professional demeanor with elderly populations
Ability to work independently with minimal supervision in a home care environment
