Horizon House is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania’s oldest psychiatric rehabilitation agency. Since 1952, Horizon House has been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to assist them to live as independently as possible in a community-based setting. With over 100 locations throughout Southeastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.

We are expanding our team and are seeking qualified candidates to join our Residential Service Group Home team at our location in Milton, Delaware.

Work Schedule:

The position is scheduled for 40 hours per week, consisting of two 8-hour shifts. Work schedules are developed based on program and operational needs.

• Preferably available to work every other weekend (Saturday and Sunday) on a rotating schedule.
• During the alternate weeks when not scheduled to work the weekend, incumbent will be assigned two additional weekday shifts to maintain a consistent full-time schedule and 8-hour shift structure.
• Candidates must be available to work one of the following shifts: 4:00 PM - 12:00 AM, 12:00 AM - 8:00 AM

The Residential Clinician will provide coordination and guidance, counseling, and encouragement to participants as they transition from residential services to a more independent environment. The Residential Clinician will also implement and document participant’s progress in accordance with individualized rehabilitation plans and ensure a safe, healthy and secure environment which promotes growth and development.

Some additional responsibilities would include the following:

• Responsible for case management/service coordination of all assigned participants to include chart compliance, continuity of care, person centered planning and skill development for participants.
• Complete the initial and annual participant assessments and develop the participant’s recovery plan from a person-centered perspective.
• Assist with the presentation in case conferences for assigned participants. Assist with the analysis and identification of service delivery gaps.
• Act as a liaison and consult with community agencies and families to maintain coordination in the recovery process.
• Provide ongoing assessment of participant’s mental illness symptoms and their responses to recovery to ensure that immediate and appropriate interventions are provided.
• Participate in staff planning, training and internal/external administrative meetings.

Qualifications:

• A master’s degree in clinical or counseling psychology, clinical social work, vocational/psychiatric rehabilitation , a registered nurse with certification in mental health from the American Nursing Association; or a person with a bachelor’s degree with five (5) years experience in mental health service delivery with at least two (2) years experience in residential services
• Experience in mental health or social services with an emphasis on direct care in community-based settings.
• A demonstrated ability to identify participant’s strengths and to assist participants with learning to benefit from those strengths.
• Proficiency in verbal and written communication.
• Ability to work independently and to exercise initiative.
• A professional and personal rehabilitation philosophy supported by employment experience that demonstrates compatibility with psychosocial rehabilitation and person centered planning principles and standards.

Scope:

• Requires the ability to lift, carry, fold, bend, kneel, stack, stoop, which includes direct intervention with participants.
• Frequent talking and listening.
• Ability to climb and descend stairs.
• Possess manual dexterity and fine motor skills.
• Requires crisis intervention in emergency situations.
• May be required to report to work during emergencies including inclement weather.
