Hospice RN Case Manager

What You'll Need:

• Valid RN licensure with active state registration
• Proven hospice care experience, including familiarity with the HCHB electronic medical record system
• Strong clinical assessment skills encompassing physical, psychosocial, environmental, safety, and developmental needs
• Ability to develop, implement, and evaluate individualized care plans
• Excellent communication skills to educate patients, families, and caregivers
• Ability to collaborate effectively within an interdisciplinary team
• Knowledge of pain management, symptom control, and end-of-life care standards

Nice to Have:

• Bilingual abilities or multicultural experience to support diverse patient populations
• Additional certifications such as CPR, Hospice/Palliative Care certifications

Preferred Education and Experience:

• Bachelor's degree in Nursing (BSN) or higher preferred
• Minimum of 2 years of hospice or palliative care experience
• Proficiency with electronic health records and hospice-specific software

Other Requirements:

• Must possess reliable transportation for occasional patient/family visits
• Ability to work Monday through Friday from 8:00 AM to 5:00 PM
• Commitment to maintaining compliance with all regulatory, legal, and ethical standards
