Licensed Practical Nurse (LPN) – Home Health/Hospice

Schedule: 8 hours per shift

Assignment Details:

• Contract Length: 14 weeks
• Guaranteed Hours: 40 hours per week

Requirements:

• Current Nursing License in the practicing state
• Valid Driver's License
• Current CPR Certification
• Two years of experience as an LPN in a clinical setting, preferably in a home health or hospice setting
• Proficiency in electronic medical charting and time-keeping software

Roles & Responsibilities:

• Administer skilled nursing care to patients in their residence as prescribed by a physician and in accordance with the established plan of care.
• Assist in identifying patients' physical, psycho-social, and environmental needs, and report outcomes to the supervising RN.
• Participate in planning, coordinating, and implementing care in conjunction with the RN to provide quality patient care.
• Provide effective communication to patients/families and co-workers, properly documenting outcomes of interventions.
• Assist patients and families in learning appropriate self-care techniques and assess their responses to care.
• Ensure care is provided utilizing infection control measures.

Compliance / Notes:

• Proof of current Hepatitis consent/declination and TB vaccination record is required.
• Must read, write, and speak fluent English.
• Must have good and regular attendance.
• Additional duties may be assigned as needed.
