Nurse Practitioner

Nurse Practitioner Location: Gulfport, MS Join a collaborative, patient-centred care team where you can build meaningful relationships, provide high-quality longitudinal care, and make a lasting impact on the lives of older adults. This role offers the opportunity to work alongside Physicians, Care Coordinators, Medical Assistants, and Nurse Care Managers to deliver comprehensive, value-based care in both clinic and home settings. What You'll Do Provide comprehensive primary care services to adult and geriatric patients. Conduct annual wellness visits, post-discharge follow-ups, and chronic disease management assessments. Perform in-home and clinic-based evaluations, including physical exams, medication reviews, vital sign assessments, lab draws, wound care, and depression screenings. Collaborate with physicians, specialists, and care facilities to coordinate patient care. Identify care gaps and communicate findings to the care team to improve patient outcomes. Educate patients and families on disease management, medications, preventive care, and treatment plans. Deliver compassionate, relationship-focused care that improves patient engagement and quality of life. Maintain accurate documentation and ensure compliance with HIPAA and regulatory standards. Qualifications Active Nurse Practitioner license. Minimum 2 years of NP experience in Primary Care, Adult Medicine, or Geriatrics. Experience in an outpatient setting. Knowledge of HCC documentation, ICD-10 coding, and Health Risk Assessments. Current DEA and BLS certifications.
