Service Facilitator (SF) Case Manager with Moms In Motion in Virginia is seeking empathetic professionals to support families navigating Virginia’s Medicaid Waiver programs (CCC+, CL & FIS, EPSDT). Daily local travel to client homes is required, with the flexibility to work remotely from a laptop or tablet.
