This role is ideal for an LPN experienced in long-term care or rehabilitation settings who can work independently while supporting resident care. Responsibilities Provide direct nursing care in compliance with facility policies and state regulations. Administer medications and treatments safely and accurately. Monitor patient conditions and report any changes to the supervising RN or provider. Maintain timely and accurate documentation of care provided. Collaborate with interdisciplinary team members to ensure quality care. Requirements Active LPN license in the state of Vermont. Prior experience in long-term care, rehabilitation, or skilled nursing preferred. Ability to work one scheduled day per week for the duration of the contract.
