Position Details Department: PCH‑SW | Social Work Location: Phoenix Shift: Mon‑Fri, Days, 8am‑5pm Category: Care Management/Social Work/Case Management Posting #: 1030862 Employee Type: PRN Position: Social Worker I Position Summary This position provides clinical assessment, therapeutic interventions, care coordination, transition planning and emotional support to patients and families at Phoenix Children’s within their scope of practice. The social worker participates as an interdisciplinary team member, delivering interventions that include biologic, psychosocial assessment, diagnosis, treatment planning, therapeutic interventions, crisis interventions, coordination of care, bereavement and transition/discharge planning. The role requires knowledge of family systems and interpersonal dynamics; mandated reporter responsibilities; use of financial, community and other resource networks; coordination with DCS, police jurisdictions and other governmental authorities; the ability to advocate with outside systems; and collaboration with medical staff. Psychosocial Assessments and Interventions Assess the bio‑psychosocial status, including mental health status, diagnostic impressions, support systems, safety issues, risk for abuse/neglect, suicide risk assessment, evaluate clients for placement appropriateness, coping skills, family systems, and cognitive abilities. Identify therapeutic goals to improve family functioning, mental health, goal attainment, self‑sufficiency, medical adherence. Identify barriers to treatment and recovery, and help patients/families build strengths and protective factors to facilitate successful discharge. Documentation Complete clinical record documentation in a timely manner and provide information to coordinate care, confirm demographics, and recommend disposition plans. Complete handoff documentation to the Social Work Department per procedures. Coordination of Care Develop and coordinate effective treatment interventions and comprehensive treatment plans. Maintain effective communication with medical staff, nursing staff, case managers, peers and families. Develop treatment goals and objectives across disciplines, with discharge planning. Attend care conferences, interdisciplinary team rounds, and critical incident/stress debriefings as needed. Monitor patients during transition between outpatient and inpatient services. Refer cases requiring risk management, peer review, or quality intervention to appropriate units. Participate in hospital and departmental operations initiatives to improve care delivery. Conduct or facilitate individual, group, and family therapy meetings. Review open patient records for policy compliance and service delivery. Develop home contracts, level contracts, goals and objectives. Education Maintain professional competency through educational programs, professional publications, community committees, and professional societies. Maintain current knowledge of community resources. Transplant social workers receive ongoing education in transplantation. Participate fully in clinical supervision as required. Miscellaneous Duties Perform miscellaneous job‑related duties as requested. Position Qualifications Education Master’s in Social Work from an accredited institution (Council of Social Work Education) or equivalent foreign degree. Experience Child protection experience (required). Three years post‑master’s social work experience in health or behavioral health setting (preferred). Pediatric social work experience (preferred). Special Skills Excellent communication skills, both verbal and written. Effective decision‑making, problem‑solving skills, creativity, patient and family strengths, influential leadership skills. Critical thinking skills and ability to anticipate patient discharge needs. Ability to assess symptoms and develop diagnostic impressions and treatment plans. Moderate to expert computer skills. Additional Requirements Current State of Arizona Licensed Master in Social Work by the Arizona Board of Behavioral Health Examiners. CPR / First Aid and CPI training or ability to obtain at hire. Valid AZ Department of Public Safety Fingerprint Clearance Card (preferred). Certification / License / Registry Requirements Licensed Master Social Worker – Required.
