Registered Nurse Home Health

Job Summary:

This Home Health Registered Nurse position involves delivering patient-centered care to primarily geriatric populations in Carroll County, with support for Baltimore County. The role requires assessing, planning, and implementing nursing care plans to promote health and well-being.

Responsibilities:

• 
  Provide skilled nursing care to patients according to individualized plans of care.
  
  
• 
  Complete and document OASIS assessments, admission, recertification, and routine visits within established timeframes.
  
  
• 
  Administer medication, manage pain, and monitor blood glucose levels.
  
  
• 
  Perform wound care procedures including pressure ulcer management, wound VACs, and assist with IV therapy and infusion treatments.
  
  
• 
  Educate patients and caregivers on medication management, ostomy, urinary catheter, and feeding tube care.
  
  
• 
  Submit visit notes and documentation within 2448 hours of each visit.
  
  

Qualifications:

• 
  Possess 23 years of recent skilled home care experience, including OASIS documentation.
  
  
• 
  Hold Maryland or compact RN license.
  
  
• 
  Maintain BLS certification.
  
  
• 
  Have a valid drivers license and reliable vehicle.
  
  
• 
  Demonstrate proficiency in wound care, IV therapy, and medication administration.
  
  
• 
  Be able to work MondayFriday, with potential weekend coverage and on-call duties as needed.
