Hospice Nurse Practitioner / Palliative Care Nurse Practitioner

This role offers the opportunity to make a profound impact across the Piedmont Triad region (Greensboro, Winston Salem and High Point) by supporting patients and families during life’s most meaningful moments.

About You
Be able to do the job as described
Experienced Nurse Practitioner seeing patients in office or home setting (no new grads)
Some experience with hospice or palliative care is a plus
Active Nurse Practitioner license in North Carolina (or ability to obtain)
Strong ability to work independently in a home care setting
What You’ll Be Doing
Provide in-home hospice and palliative care to patients
Conduct patient assessments and develop care plans focused on comfort
Support and educate patients and families on end-of-life care and advanced directives
Collaborate with physicians, consultants, and interdisciplinary care teams
Ensure delivery of high-quality, compassionate hospice services
