If you are passionate about helping patients remain safe, comfortable, and independent while working alongside experienced nurses and healthcare professionals, we'd love to hear from you.This is an excellent opportunity for Home Health Aides seeking a flexible Per Diem / Per Visit schedule while making a meaningful impact in the lives of patients and their families.The Home Health Aide provides personal care and related services in the patient's home as permitted under California law.Services are performed under the direction and supervision of the Nurse Case Manager, Registered Nurse, and Director of Patient Care Services in accordance with each patient's individualized Plan of Care (POC).Patient visits are assigned based on physician orders, patient needs, geographic location, and caregiver availability.As a Home Health Aide, you will:Provide personal care services according to each patient's individualized Plan of Care (POC)Assist patients with Activities of Daily Living (ADLs)Assist patients with Instrumental Activities of Daily Living (IADLs)Assist with bathing, dressing, grooming, toileting, and personal hygieneAssist patients with ambulation and mobilityMaintain a clean, safe, and healthy home environmentPrepare nutritious meals when directed in the patient's Plan of CareAssist with treatments delegated by the supervising Registered NurseAssist therapy personnel with rehabilitative activities as assignedProvide medication reminders for medications ordinarily self-administeredObtain and document vital signsEncourage patients to remain as independent as possibleProvide emotional support to patients and family membersComplete accurate and timely documentation for every patient visitImmediately report changes in the patient's condition or home environment to the supervising Registered NurseParticipate as a member of the interdisciplinary care teamAttend required in-service education and trainingCoordinate patient visits with the clinical supervisorNotify the agency promptly regarding absences or scheduling conflictsWear company identification while providing patient carePerform other duties as assigned.Applicants must meet one of the following qualifications:Current California Certified Home Health Aide (HHA)Successful completion of a CMS-approved Home Health Aide Training and Competency Evaluation ProgramSuccessful completion of a State-approved Nurse Aide Training Program meeting federal requirementsState licensure meeting CMS Home Health Conditions of ParticipationAdditional Requirements:Current CPR CertificationValid California Driver's LicenseReliable, insured vehicleReliable transportation to patient visitsAbility to read, write, and communicate effectively in EnglishSuccessful completion of required background screeningAbility to work independently while following the patient's Plan of CareProfessional, dependable, and compassionate approach to patient careExperience in any of the following is preferred:Home HealthSkilled Nursing SupportDementia & Alzheimer's CareHospice & Palliative CarePost-Acute RecoverySafe Patient TransfersFall PreventionVital SignsElectronic Medical DocumentationSchedule:Per DiemPer VisitFlexible SchedulingDaytime VisitsMonday–FridayWeekend Availability (as needed)Work Environment:This position requires travel throughout Solano County to provide care in patients' homes.Home Health Aides work independently while maintaining close communication with the supervising Registered Nurse and clinical team.Potential exposure to communicable diseases may occur.
