Basic Function: Assist in supervising and support for direct support professionals who are working in the day habilitation without walls program and are out in the community. 

Essential Functions:

• Provision of direct support to individuals in day habilitation program 
• Accurately and timely submits paperwork to the Habilitation Supervisor 
• Keeps current with each individual’s Day Habilitation Plan and documentation forms to assure program activity relates to the consumer’s plan 
• Cooperates with the Habilitation Supervisor and the consumer’s Medicaid Service Coordinator to assure Habilitation Plans are updated for ISP reviews and meetings 
• Be professional in appearance, attire, and attitude while in the consumer’s home and in the community 
• Complies with agency policy 
• Provides additional supports for other DSPs in implementation of habilitation plans 
• Assists in ensuring staff is utilizing all required documentation. 
• Maintains compliance with institutional regulations, state and federal regulations. 
• Takes the lead role while in the community providing Day Habilitation for individuals, which may include assisting in coordination of community activities and volunteer opportunities in conjunction with Habilitation Supervisor. 

Additional Functions:

• Adheres to safety policies and procedures. Insures that clients/students/program participants receive services in a safe environment. Encourages co-workers to follow safety policies and procedures. Reports any potential unsafe conditions to supervisor and/or Safety Committee. 
• Follows current policies and procedures regarding compliance with privacy, security and corporate compliance policies. Be thoroughly familiar with department and program specific policies and procedures. 
• Maintains positive and open communication with supervisors. 
• Attends meetings as requested by the Supervisor.

Qualifications: Associates Degree in Human Services Area and 1year experience working with individuals with developmental disabilities; or some college credit and 2 years’ experience working with individuals with developmental disabilities: or High School Diploma or GED and 3 years’ experience working with individuals with developmental disabilities. 

Special Skills, Knowledge and Abilities: Be professional in appearance, attire, attitude and communication, Ability to work collaboratively with others in a team environment; or work independently without close supervision, Ability to communicate with individuals from varied backgrounds, including individuals with cognitive challenges and professionals from within and outside Inspire. Ability to communicate effectively with others using oral and written methods, Ability to remain calm and exercise sound judgment when engaged in crisis intervention; highly developed verbal and written skills. 

Physical Demands: While performing the duties of this job, the employee is regularly required to sit and/or stand; use hands to finger, handle, or feel; reach with hands and arms; and talk or hear. The employee frequently is required to walk and stoop, kneel, or crouch. The employee must occasionally lift and/or move up to 10 pounds and occasionally lift and/or move up to 50 pounds. Normal or corrective vision is required. Specific vision abilities required by this job include close vision, depth perception, and ability to adjust focus. 

Environment: The noise level is usually low to moderate.
