Continuum Therapy Partners is seeking a compassionate and dedicated Occupational Therapist (OT) to join our rehabilitation team at Helia Healthcare of Energy in Energy, Illinois.

This full-time Occupational Therapist opportunity offers the chance to provide exceptional occupational therapy services in a supportive, collaborative skilled nursing facility where your expertise makes a meaningful impact every day.

As an Occupational Therapist (OT) , you will work with adult and geriatric patients to improve independence, safety, and quality of life through individualized occupational therapy interventions. You will collaborate with an interdisciplinary rehabilitation team focused on achieving positive patient outcomes.

Responsibilities

• Evaluate patients and develop individualized occupational therapy treatment plans.
• Provide skilled occupational therapy services to improve functional independence and quality of life.
• Address activities of daily living (ADLs), mobility, coordination, strength, cognition, and safety awareness.
• Assist patients in achieving their goals related to independence and daily function.
• Collaborate with physicians, nursing staff, caregivers, and interdisciplinary team members.
• Complete accurate and timely documentation according to company, state, and federal guidelines.
• Educate patients and caregivers on therapeutic techniques, adaptive equipment, and safety strategies.
• Deliver compassionate, evidence-based occupational therapy services.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Illinois Occupational Therapist (OT) license required.
• Skilled nursing or rehabilitation experience preferred but not required.
• New graduates are encouraged to apply.
• Strong communication, organization, and teamwork skills.
• Passion for providing high-quality, patient-centered care.
