Quality Assurance Performance Improvement Coordinator

Hartford Healthcare is seeking a Quality Assurance Performance Improvement Coordinator RN to drive quality and patient safety within our HomeCare services. In this role, you will lead QA/PI initiatives, conduct chart audits, analyze clinical and operational data, and monitor key performance indicators to ensure compliance with regulatory and accreditation standards. You will collaborate with interdisciplinary teams to develop action plans, update policies, and provide staff education, supporting a culture of continuous improvement, teamwork, and compassionate, high-quality home-based care across our Connecticut network.
