About the Position

Job Title: Physical Therapist Profession: Rehabilitation Therapy Specialty: Physical Therapist Duration: 13 weeks Shift: M - F 8am to 4pm with 2-3 weekend days required Hours per Shift: 8 Experience: Minimum 2 years License: Current physical therapy license Certifications: Valid CPR certification Must-Have: - Comprehensive knowledge of physical therapy assessments and treatments - Ability to develop and implement effective care plans - Strong communication and coordination skills Description: Actively monitors and amends patient visit schedules to maximize productivity and meet patient needs. Takes action to account for new patients and changes in existing schedules. This includes calling patients to reschedule visits and reshuffling patient load. Communicates recommended schedule changes to supervisor. Performs comprehensive assessments of assigned patients, including psychosocial and safety factors. Documents assessments in accordance with federal and state regulations and standards of care. Develops a plan of care to promote optimal functional independence and safety. Creates patient-specific goals and ensures that the plan reflects logical interventions. Plans for and provides skilled physical therapy treatment, including therapeutic techniques and education. Assesses patient status and progress on each visit, ensuring quality of care is documented. Coordinates patient care with other clinical team members to ensure optimal communication and care. Supervises and directs clinical activities of assistants, students, and aides. Completes and coordinates required paperwork to ensure compliance with regulations and standards. Ensures documentation reflects care provided and outcomes achieved. Completes required paperwork for accurate charging and billing processes. Participates in training programs for various staff and community groups. Maintains and enhances personal knowledge and promotes standards in physical therapy. Engages in marketing and promotion of rehabilitation services. Carries out a variety of patient-related assessments and procedures. Communicates changes in patient care schedules to scheduling personnel in a timely manner. Completes required documentation in compliance with guidelines, regulations, and policies. Carries out visits in a time-effective manner and communicates with scheduling personnel as necessary. Assesses patients' needs for home care services and coordinates plans of care with patients and caregivers. Coordinates safe patient discharges and transfers, ensuring appropriate paperwork is processed. Transmits patient incident reports to appropriate personnel in a timely manner. Participates in regular peer review audits, providing and accepting constructive feedback.
