About the Role

As a Qualified Professional (QP) at Golden Age Foundation, you’ll play a vital role in bridging the gaps between mental health, reentry, family support, education, employment, housing, life skills, and community resources. You’ll work closely with individuals to help them heal, rebuild, and thrive, all while contributing to our vision of generational progress and stability.

This position is ideal for someone with at least one year of experience who is ready to take their skills to the next level and be part of a groundbreaking approach to community care.

Key Responsibilities

As a Qualified Professional, you’ll:
- Collaborate with individuals, families, and communities to assess needs and connect them to the right resources at the right time.
- Develop and implement personalized care plans that address mental health, housing, employment, education, and other essential areas.
- Act as a key liaison between clients and community resources, ensuring seamless access to support services.
- Monitor progress and adjust care plans as needed to ensure long-term stability and success.
- Work as part of a collaborative team to build an interconnected ecosystem of care and opportunity.

Required Skills and Qualifications

To excel in this role, you’ll need:
- A minimum of 1 year of relevant experience in a related field.
- Strong interpersonal and communication skills to connect with individuals from diverse backgrounds.
- A proactive and compassionate approach to problem-solving.
- The ability to work collaboratively as part of a team.
