Key Responsibilities\n

• Deliver education and resources to patients and their families regarding community support and post-rehabilitation services.

• Participate in discharge planning, facilitating smooth transitions for patients returning home or relocating to other care settings.

Patient Population / Client Focus\n

The Licensed Clinical Social Worker will work with a diverse patient population, including individuals recovering from strokes, neurological disorders, orthopedic surgeries, and other complexities associated with physical rehabilitation.
