Job Summary:

Join our team of compassionate and dedicated healthcare professionals at Beachside Post Acute, a warm and welcoming facility that specializes in compassionate care and rehabilitation. As a Physical Therapist, you will play a critical role in helping our residents achieve their highest level of mobility and independence. We are seeking a licensed Physical Therapist with a passion for providing high-quality care to join our per diem team.

Responsibilities:

• Evaluate residents' physical abilities, functional limitations, and mobility needs using standardized assessments

• Develop and implement individualized treatment plans based on assessment results, resident goals, and physician orders

• Provide skilled physical therapy interventions, including therapeutic exercise, manual therapy, and modalities

• Monitor and document resident progress and adjust treatment plans as necessary

• Communicate with interdisciplinary team members to coordinate care and ensure optimal outcomes for residents

• Educate residents and family members on physical therapy goals, treatment plans, and home exercise programs

• Maintain accurate and timely documentation of physical therapy services provided

• Participate in team meetings, in-service education, and quality improvement initiatives

• Maintain current knowledge of physical therapy practice and relevant regulations

Qualifications:

• Current license to practice as a Physical Therapist in the state of California

• Minimum of 2 years of experience in a healthcare setting, preferably in a skilled nursing facility

• Knowledge of current physical therapy practice standards and regulations
