Now Hiring: Hospice Patient Care Manager RN –Valparaiso, Indiana

Full-time with supervisor on-call rotation

Our hospice organization is seeking a dedicated Patient Care Manager (PCM) RN to guide our interdisciplinary team and ensure exceptional care for every patient we serve.

What You’ll Do

• Lead and mentor a team of nurses, aides, social workers, and chaplains
• Oversee patient care plans and ensure regulatory compliance
• Support families with empathy, clarity, and professionalism
• Collaborate closely with physicians and clinical leadership
• Foster a culture of compassion, accountability, and excellence

What We’re Looking For

• RN license in the state of Indiana
• Hospice or home health leadership experience preferred
• Strong communication and organizational skills
• A heart for service and a commitment to quality care
