This opportunity uses Home Care Home Base EMR and supports clinicians with orientation and productivity expectations.

Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Occupational Therapy from an accredited educational program.
• Active Illinois Occupational Therapy license required to start the assignment.
• BLS certification may be required from AHA or ARC.
• Experience using Home Care Home Base preferred.
• Productivity expectation: 30 visits per week for registered therapists.
• Orientation: 5 days required.
• Valid driver's license and ability to travel to patient homes; expected mileage varies, around 25 miles per day.
• Prior home health experience preferred; open to candidates new to the setting.

• Schedule: Monday through Friday.
• EMR: Home Care Home Base.
• Orientation period: 5 days.
• Productivity target: 30 visits per week for registered therapists.
• Candidates will travel to patient homes; typical daily travel approximately 25 miles.
