TARGETED CASE MANAGER

POSITIONSUMMARY:

Providesservicecoordinationtotargetedpopulationclients(Childor Adult).

QUALIFICATIONS:

Bachelor’s degree in psychology, social work, or mental health; preference for work experience with the targeted population assigned within the human service setting. Minimum one (1) year of experience post degree working with persons in a human services setting. Must provide own transportation to provide home visits.

SKILL SETS:

Adequate communication skills, process verbal and written instructions, dependable, interact effectivelywiththepublicandstaff,adaptability-abletodealwithfrequentchange,delaysorunexpectedevents, strong sense of commitment.

PHYSICAL AND ENVIORMENTAL DEMANDS:

Visual acuity necessary for updating files and completing documents. Requiresfrequentstooping,kneelingandcrouchingto filerecords. Hearingandtalkingisnecessary toanswerphoneandtalkwithemployeesandotherindividuals. Workislightwithwalking,standingandsitting. Manual dexterity is required to perform work processing and other clerical duties. Possible exposure to blood borne pathogens, biohazardous waste, hazardous waste and communicable diseases.

JOBSPECIFIC FUNCTIONS:

1. Conductshomevisits toassessservicecoordinationneedsfor clients.

1. Assistsclientsinaccessingallneededserviceswithinthe community
2. Performsadvocacyon behalf of clients
3. Providescase consultationstoservice providers
4. Provide pre-tenancy and tenancy supports to assist participants in identifying, selecting, and entering into a lease agreement resulting in the participant moving into an independent housing unit.
5. Performsotherdutiesas assigned.
