We are dedicated to improving the quality of life for our residents through personalized care and support. Description Job Description As a Licensed Practical Nurse (LPN) working with IntensaCare, you will be responsible for providing personalized, one-on-one care to individuals with developmental disabilities. You will ensure that each patient receives high-quality, person-centered care, and work closely with their family, and our team of healthcare professionals. Each shift, you will be supported by a Certified Nursing Assistant (CNA) or Direct Support Professional (DSP), who will be present in the home to assist you. Key Responsibilities : Provide direct, 1:1 patient care for individuals with developmental disabilities (DD) under the supervision of a Registered Nurse (RN). Administer medications, treatments, and prescribed procedures. Monitor and assess patients' vital signs and health status, reporting any changes to the RN or physician. Assist with daily living activities, including bathing, dressing, feeding, and mobility. Maintain accurate and timely documentation of the individual's care, observations, and interventions. Collaborate with the health care team to develop, implement, and evaluate individualized care plans. Ensure compliance with all state and federal regulations and company policies. Qualifications : Valid and active LPN license in Virginia. Minimum of 1 year of nursing experience preferred, particularly in home health, private duty nursing, or a similar setting. Experience working with individuals with developmental disabilities (DD) is a plus. Current CPR certification. Strong clinical and assessment skills. Excellent communication and interpersonal skills. Ability to work independently and manage time effectively. Compassionate, patient-focused, and reliable.
