Crisis Case Manager: Care Coordinator & Advocate

Connections Health Solutions in Virginia is seeking a Case Manager to support crisis care patients. You will manage admissions, treatment and discharge planning, coordinate with an Interdisciplinary Treatment Team, and maintain detailed records.
Qualifications include a Bachelor's degree in behavioral health or related field and 2 years of experience, QMHP, and ability to pass background checks. We offer shift differentials and comprehensive benefits for full-time staff.
