About This Role As a PRN RN Care Coordinator at Cleveland Clinic, you will collaborate with multidisciplinary caregivers to provide care coordination and disease management for patients with chronic conditions. Highlights Location: West Palm Beach, FL Schedule: Per Diem, Day shift What You'll Do Work collaboratively with a multidisciplinary care team for high-risk patients. Provide care and disease management coordination. Identify patients with ongoing coordination needs and conduct targeted outreach. Outline involvement needed by the specialty care team and primary care team. Utilize assessment skills and risk assessment tools for patient care needs. Utilize technological tools to manage patient populations. Conduct comprehensive clinical assessments of patients. Inform and work with patients and families regarding care coordination. Serve as a liaison and advocate for patients and families. Assist in managing transitions of care across settings. Requirements Education: Graduate from an accredited school of Professional Nursing Experience: 3-5 years of nursing experience Licenses & Certifications: Current state licensure as a Registered Nurse (RN) Basic Life Support (BLS) certification required Preferred: Bachelor’s of Science in Nursing (BSN) Specialty certification Outpatient and/or Emergency Department experience Advanced Cardiac Life Support (ACLS) certification preferred Care coordination experience About Cleveland Clinic Cleveland Clinic is a leading healthcare provider committed to delivering exceptional patient care.
