Sickle Cell Inpatient NP/PA - Acute Care Coordination

Northwestern Medicine seeks a Nurse Practitioner or Physician Assistant to join the Sickle Cell Disease program, with inpatient consults and a smaller outpatient clinic component. The APP will collaborate with hematology, hospital medicine, pain management, care coordination, and outpatient teams to improve continuity of care across settings. The successful candidate will help bridge inpatient, emergency, infusion, and outpatient care settings while addressing barriers to care that contribute to #J-18808-Ljbffr Northwestern Medicine
