Speech Language Pathologist - Per Diem

Northern Light Mercy Hospital | US: ME: Portland | Rehabilitative Services | PRN Description Northern Light Mercy Hospital Department: Speech Therapy Position is located: Mercy Fore River Work Schedule: Variable Summary: Under the guidance of the Manager Physical Rehabilitation and Medical Director, the Speech Language Pathologist performs evaluations and plans and implements rehabilitative treatment programs on pediatric through geriatric patients as ordered and approved by physicians. The incumbent performs duties and tasks in accordance with standards established for the job. The incumbent may have access to highly confidential patient, employee and/or Mercy proprietary information, and must handle and protect the information in accordance with hospital and system policies, HIPAA requirements and the highest level of ethical standards. The incumbent is responsible for reporting all security events, potential events, or other security risks to the organization. The incumbent is responsible for participation in and completion of all patient safety initiatives appropriate to the position, and conducts all job responsibilities according to the Mission and Values of Hospital. Responsibilities Patient Assessment: Complete an initial evaluation within 24 hours of inpatient (IP) and three working days of outpatient (OP). Select and perform appropriate evaluative methods relevant to chief complaint and history. Recognize need for potential interdisciplinary involvement and seek referral. Recognize inappropriate referral and seek therapy discontinuation. Identify pertinent speech-language pathology problems. Include rehab potential and discharge planning. Complete re‑assessments as changes in condition warrant or upon discharge per department standards. Plan of Care: Design individual plan of care based on identified therapy problems. Develop realistic, time‑based goals. Select treatment methods to achieve goals. Participate in multidisciplinary Focus of Care Rounds and/or patient/team conferences. Modify plan of care as needed based on patient response and changing needs. Re‑evaluate according to time‑based goals per department standards. Implementation of Treatment Interventions: Demonstrate competency with clinical skills and treatment procedures. Maintain a safe and therapeutic environment following infection control, patient safety, and equipment operation standards. Notify physician/other professionals of progress or changes. Conduct patient/family teaching based on identified needs. Provide written instructions at discharge. Recognize when optimum benefit reached and seek therapy discontinuation. Ensure discharge planning needs are met. Documentation: Document initial assessment, plan of care, progress notes, and discharge status timely. Include subjective findings, objective findings, therapy assessment, problem list, goals, plan of care, discharge disposition, daily progress notes, re‑evaluation, revised goals/plan, patient education, understanding of education, referrals, discharge planning, discharge documentation, and ensure signature, time and designation of therapist. Job Management: Manage typical patient volume during regular hours. Prioritize caseload to accomplish tasks. Maintain expected productivity per department standards. Complete stats sheets and charges each day. Delegate tasks to assistants, aides or volunteers as indicated. Professional Practice / Leadership: Identify own learning needs and promote professional communication. Meet yearly continuing education requirements. Attend staff meetings and review minutes when absent. Participate in additional self‑growth tasks. Plan and implement self‑objectives. Participate in peer review and quality improvement indicators. Accept schedule and department changes. Participate in student program with required number of students. Volunteer for special projects and hospital‑wide committees. Expand knowledge and skills for any rotation. Serve as resource to staff. Provide in‑service for educational courses attended. Participate in orientation of new employees. Develop programs. Engage in professional organization. Team Skills: Work with healthcare team to achieve positive outcomes. Recognize interdependence and unique gifts. Communicate positively and adapt to change. Demonstrate insight into interpersonal impact. Seek group input and participate in decisions. Show respect and keep others informed. Express concerns timely. Recognize limits and seek assistance. Credentials Required Speech Pathologist Education Required Bachelor's Degree Working Conditions Potential exposure to abusive and/or aggressive people. Potential exposure to diseases or infections. Potential exposure to hazardous materials. Potential exposure to noise levels being uncomfortable. Potential exposure to noxious odors. Work with computers, typing, reading or writing. Lifting, moving and loading 20 to 30 pounds. Prolonged periods of sitting. Prolonged periods of standing. Prolonged periods of walking. Potential exposure to very hot or cold temperatures.
