Physical Therapist (PT)

Pride Health is actively hiring a Physical Therapist (PT) to join our client's growing Home Health team. Location: Farmington Hills, MI, 48334 Compensation: Local Pay: $55.00 - $70.00/hour (W-2) Travel Pay Package: Up to $2,800/week gross (includes weekly non-taxable stipend)

Key Responsibilities:

• Perform comprehensive physical therapy evaluations, reassessments, OASIS documentation, and individualized plan of care development, assessing strength, balance, mobility, pain, functional limitations, and home safety risks to establish measurable rehabilitation goals.
• Deliver skilled home health physical therapy interventions, including therapeutic exercises, gait and transfer training, balance and fall prevention, endurance training, pain management, and mobility enhancement to maximize patient independence and functional outcomes.
• Educate patients and caregivers on home exercise programs (HEP), safe mobility techniques, assistive device use, fall prevention strategies, and disease-specific management while recommending home modifications and durable medical equipment (DME) to improve safety.
• Collaborate with physicians and the interdisciplinary care team to monitor patient progress, modify treatment plans as needed, complete timely and accurate documentation in compliance with Medicare and agency standards, and efficiently manage daily travel schedules and productivity expectations across the assigned coverage area.

Weekly Hour Guarantee: 40 Productivity Expectations (points): 30 Coverage Areas: Wayne county, Oakland county, Washtenaw county, Genesee county, Livingston county, Macomb county. We do try to keep clinicians in specific regions, but everyone is expected to cover if other clinicians are on vacation or call out sick. Shift schedule (days, start & end times): Monday-Friday 8am-4:30pm On Call/Weekend Requirement: none Will Milage be reimbursed between visits, from first patient to last patient (Y/N)? Y at $0.51/mile Must have qualifications/experience:

• Doctor of Physical Therapy (DPT) degree from an accredited program
• Must be licensed in Michigan
• 1-2 years' experience in home health
• Experience with HCHB EMR system
• OASIS & documentation proficiency
• Strong clinical assessment & decision-making
• Ability to work independently in the field
• Effective care coordination with interdisciplinary teams
• Strong time management & scheduling skill

Type of certification required: BLS Dress code/scrub color: navy blue scrubs or tan/black slacks and a navy blue polo type shirt
