If your skill and compassion as an Outpatient Nurse Practitioner reflect that mission, you belong at Legacy, making a difference through patient care and administrative duties in the clinic setting.ResponsibilitiesCompletes history taking, physical examinations, diagnosis, and treatment of both chronic and episodic disorders.Assigned principal responsibility for primary health care of patientsPlans and implements health care goals for patients.Responsible for the patient throughout the course of the treatment, provides the physical examination, the diagnosis, and the treatment plan.Refers patients to specialists for consultation or to specialized health resources for treatment but retains responsibility for clinical management as well as any subsequent modification.Collaborates to care for complex social and medical needs with health care team including but not limited to behavioral health clinicians, care management and social work.QualificationsEducation:As required by licensure.Experience:Clinical experience requirements are dependent upon the specific position, and include a minimum of 2 years of experience in a related field.LicensureCurrent applicable state RN license required.National certification as a Nurse Practitioner (NP) required.
