Hospice Registered Nurse (RN)
Join our compassionate hospice team and make a meaningful difference by providing high-quality, patient-centered care in the home. We're seeking an experienced Hospice RN who is comfortable working independently while delivering exceptional end-of-life care.
Schedule:
- Friday, Saturday, Sunday & Monday
- 8:00 AM – 6:00 PM
Coverage Area:
- York and Lancaster Counties
Requirements:
- Active RN License
- Hospice experience required
- Homecare Homebase (HCHB) experience required
- Valid Driver's License, reliable transportation, and current auto insurance
- Current BLS/CPR certification
Productivity:
- 6–8 patient visits per day
Key Responsibilities:
- Complete hospice admissions, routine visits, PRN visits, and death visits
- Perform comprehensive patient assessments and develop individualized care plans
- Provide skilled nursing care, symptom management, and patient/family education
- Coordinate care with physicians and the interdisciplinary hospice team
- Complete accurate documentation in HCHB
- Monitor changes in patient condition and communicate updates to the care team
- Supervise clinical staff as needed while ensuring compliance with hospice standards and regulations
