We are looking for a compassionate Discharge Planner with clinical experience in multi-campus care for patients with mental health and addiction diagnoses. Skilled in crisis intervention, mental health assessments and documentation. Committed to providing a therapeutic environment, meticulous documentation, and collaborative care in adult.children's crisis and detoxification units. What you will do. Job Responsibilities: Provide clinical and psychosocial services to Acute Care patients by coordinating and facilitating the development of care plans. Provides through and accurate discharge planning information. Maintains high standards of accuracy and completeness across all required clinician and administrative records. Why SalusCare? OR a Master’s degree in a Human Services field; experience providing direct care services in a behavioral health setting preferred. Co-occurring experience preferred.
