Key Responsibilities

  • Conduct thorough clinical assessments and create individualized treatment plans that incorporate best practices and solution-focused strategies using telehealth platforms.
  • Utilize evidence-based therapeutic interventions, including cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), tailoring them to meet the distinct needs of patients remotely.
  • Maintain precise clinical documentation in compliance with legal and ethical standards, utilizing secure electronic health record (EHR) systems to ensure data integrity.
  • Collaborate with interdisciplinary team members such as psychiatrists, psychologists, and nurses in virtual consults to enhance the continuum of patient care.
  • Engage in regular case discussions and peer reviews, utilizing video conferencing tools to promote best practices and quality of care.
  • Educate patients and their families about mental health conditions, treatment alternatives, and community resources through virtual meetings and online tools.

Patient Population / Client Focus

In your role, you will focus primarily on serving patients in hospice care—a sensitive client population facing significant end-of-life challenges. You will provide invaluable support to patients by addressing their mental health needs and offering emotional guidance throughout their journey. In this role, you will help families navigate grief and prepare for significant transitions, offering compassionate care during vulnerable moments.

Qualifications

  • A valid license as a Licensed Clinical Social Worker (LCSW) in the state of Virginia.
  • A Master's degree in Social Work from an accredited institution.
  • A minimum of two years of clinical experience, particularly in providing therapeutic services related to hospice or palliative care.
  • Proven competence in delivering solution-focused psychotherapy and adapting therapeutic techniques to a remote context.
  • Exceptional communication abilities for establishing rapport with clients via digital platforms.
  • Strong problem-solving skills to efficiently manage remote work scenarios.

Preferred Experience
