About Us: Atara Home Care is a leading provider of home health care services, committed to delivering high-quality, patient-centered care. We are currently seeking a dedicated and compassionate Per Diem Physical Therapist to join our team.

Position Overview: As a Per Diem Physical Therapist, you will provide in-home physical therapy services to patients. Our Home Care services patients across Manhattan, Bronx, Queens, and Westchester. As a Per Diem you can choose your service borough. You will work with a diverse patient population, including those recovering from surgery, managing chronic conditions, or seeking rehabilitation after an injury.

Responsibilities:

• Conduct thorough assessments of patients' physical therapy needs.

• Develop and implement individualized treatment plans in collaboration with patients and their families.

• Provide hands-on therapy and education to patients in their home environment.

• Monitor and document patient progress, modifying treatment plans as necessary.

• Collaborate with other healthcare professionals to ensure comprehensive care.

• Maintain compliance with all regulatory and company policies and procedures.

Qualifications:

• Valid license to practice as a Physical Therapist in NY.

• Experience in a home care setting preferred but not required.

• Excellent communication and interpersonal skills.

• Ability to work independently and manage a variable schedule.

• Strong time management and organizational skills.
