About Us: Atara Home Care is a leading provider of home health care services, committed to delivering high-quality, patient-centered care. We are currently seeking a dedicated and compassionate Per Diem Physical Therapist to join our team.
Position Overview: As a Per Diem Physical Therapist, you will provide in-home physical therapy services to patients. Our Home Care services patients across Manhattan, Bronx, Queens, and Westchester. As a Per Diem you can choose your service borough. You will work with a diverse patient population, including those recovering from surgery, managing chronic conditions, or seeking rehabilitation after an injury.
Responsibilities:
Conduct thorough assessments of patients' physical therapy needs.
Develop and implement individualized treatment plans in collaboration with patients and their families.
Provide hands-on therapy and education to patients in their home environment.
Monitor and document patient progress, modifying treatment plans as necessary.
Collaborate with other healthcare professionals to ensure comprehensive care.
Maintain compliance with all regulatory and company policies and procedures.
Qualifications:
Valid license to practice as a Physical Therapist in NY.
Experience in a home care setting preferred but not required.
Excellent communication and interpersonal skills.
Ability to work independently and manage a variable schedule.
Strong time management and organizational skills.
