Join Our Team at Cedar Healthcare Center – Part-Time Social Worker

Cedar Healthcare Center, a senior living and rehabilitation community in Portsmouth, NH, is seeking a compassionate and dedicated Social Worker to join our team.

Position : Social Worker

Schedule : Part-time

Location : Portsmouth, NH

Pay Range : $20.00 - $25.00 per hour

What You’ll Do :

• Provide emotional and psychological support to residents and families throughout their stay

• Assist with admissions, preadmission screening processes, and resident transitions into the facility

• Participate in care planning and contribute social services goals and interventions to resident care plans

• Coordinate safe discharge planning and facilitate community resource referrals and follow-up services

• Advocate for resident rights, dignity, and quality of life

• Assist residents and families with advance care planning and end-of-life discussions

• Address resident concerns, grievances, and psychosocial needs in a timely and compassionate manner

• Help identify non-pharmacological approaches that support residents’ mental well-being

• Maintain accurate and timely documentation in accordance with facility policies and regulations

• Collaborate with interdisciplinary team members to ensure holistic resident care

• Participate in QAPI, survey readiness, and compliance initiatives as needed

What We’re Looking For :

• Degree in Social Work or related field preferred

• Prior experience in long-term care, skilled nursing, rehabilitation, or healthcare social services preferred

• Knowledge of discharge planning, resident rights, and community resources

• Strong communication, organizational, and interpersonal skills

• Ability to maintain confidentiality and professionalism in sensitive situations

• Compassionate, resident-centered approach to care

• Ability to multitask and adapt in a fast-paced healthcare environment

• Experience with EMR systems preferred

• Basic computer proficiency
