Assignment Details

• Location: Willits, CA

• Setting: Home Health

✅ Requirements

• Home Health experience – Required

• Active California Physical Therapist License – Required

• BLS/CPR Certification – Required

• Valid Driver’s License – Required

• Must be 21 years of age or older

• Master’s Degree in Physical Therapy – Preferred

Essential Responsibilities

• Conduct comprehensive patient evaluations to assess functional limitations and rehabilitation needs

• Develop and implement individualized treatment plans tailored to each patient’s goals

• Address clinical concerns identified during assessments and adjust care plans as needed

• Maintain accurate, timely documentation in accordance with regulatory and agency standards

• Ensure compliance with state, federal, and organizational guidelines

• Collaborate with interdisciplinary healthcare teams to support optimal patient outcomes
