Growing together.This position follows a hybrid schedule with three in-office days per week.Primary Responsibilities:Assist in the care of patients through telephonic outreach to identify care needs and barriers to care, connect patients to community resources, and schedule hospital follow up appointments and AWV's as neededObtain pertinent medical records for discharged patients to support provider decision making during HFU appointmentsComplete medication review of new and changed medications for patients discharged from a facilitySupport and initiate referrals and tracking of patients to internal and external patient care support programsCoordinate with post-acute services to ensure appropriate patient support has been initiatedEscalate concerns and urgent items to providers for timely resolution to support optimal health outcomesSubmit charges/create billing encounters for patient outreach to support gap closureUtilize tools and reports within the EMR to identify high risk patients and support connecting those patients to appropriate care to minimize hospitalizations and poor outcomesDocument patient interactions accurately in appropriate systems/logs and ensure compliance with relevant procedures and regulations which support provider documentation of TCM metricsDemonstrate understanding of relevant computer systems and software applications, and ensure the working condition and maintenance of necessary equipmentWork as part of a team and collaborate effectively with providers, colleagues and managementPositions in this function assist in the care of the patient by following through on Provider requests for coordination of care related to hospital dischargeIndividuals in this function typically have certification or are working towards their certification as a Medical AssistantWorks on simple tasks using established proceduresAbility to work as part of a team and collaborate effectively with others - asking for guidance when necessaryKnowledge & Personal Attributes:Knowledge of internal company protocols, procedures, and workflowsDemonstrates integrity and ethical behaviorFocuses on exceeding customer expectations and improving the patient experienceAdapts to change and contributes innovative ideasMakes fact-based decisions and delivers high-quality resultsYou'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:High school graduate or GEDCompletion of a Medical Assistant program, or Military Medic programCurrent CPR and / or BLS certification or ability to obtain certification within 30 days of hireCompetence in using relevant computer systems and software applications (e.g., MS Office Suite & Epic)Proficiency in medical terminology and patient safety proceduresProven solid communication skills to translate and communicate medical information clearly to patients and colleaguesPreferred Qualifications:Certification as a Medical Assistant or actively working towards it5+ years of experience in a healthcare setting - preferably as a Medical Assistant or Care CoordinatorExperience with EPIC EMRUnderstanding of relevant state and federal regulations (e.g., HIPAA) and scope of practiceFamiliarity with various insurance programs/products (e.g., Medicare, Medicaid)Bilingual - SpanishPay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits re subject to eligibility requirements). The hourly pay for this role will range from $16.00 to $29.00 per hour ($19.00 to $25.00 for candidates in Scottsdale, AZ) based on full-time employment.
