About the Role

Precious Hospice is seeking a compassionate, dependable, and personable Hospice Certified Nursing Assistant (CNA) to provide hands-on care and support to patients and families throughout the Georgia market. This role is responsible for assisting patients with activities of daily living while promoting comfort, dignity, safety, and quality of life throughout their hospice journey .

The CNA will provide care in patients’ homes and healthcare facilities , including skilled nursing and assisted living communities. The ideal candidate is compassionate, attentive, and committed to providing exceptional patient care while building trusting relationships with patients, families, caregivers, and the hospice care team. The CNA will also observe and communicate changes in the patient’s condition to the appropriate members of the interdisciplinary team to ensure patients receive timely and personalized care.

What You’ll Do

• Demonstrates competency in all areas of the Certified Nursing Assistant (CNA) skills competency checklist.
• Provides compassionate, hands-on personal care to hospice patients under the direction of the Director of Nursing and RN’s and in accordance with the patient’s individualized plan of care.
• Assist patients with activities of daily living, including bathing, grooming, dressing, toileting, feeding, and other personal care needs.
• Assist patients safely with transfers, ambulation, positioning, and mobility using appropriate techniques and equipment.
• Performs assigned procedures and patient-care tasks for which the CNA has been appropriately trained and has demonstrated competency.
• Observes patients for changes in condition, behavior, comfort level, skin integrity, or environment and promptly reports significant changes to the Director of Nursing and RNs.
• Accurately documents patient care, observations, and services provided in accordance with company policies and documentation requirements.
• Demonstrates proper body mechanics and safe patient-handling techniques to protect both the patient and caregiver.
• Demonstrates safe techniques for positioning and range-of-motion activities as appropriate to the patient’s plan of care.
• Maintains appropriate infection prevention and control practices in accordance with Precious Hospice policies and applicable standards.
• Maintains patient privacy, dignity, confidentiality, and respect while providing care.
• Communicate effectively with patients, families, caregivers, and members of the interdisciplinary hospice team.
• Participates in team communication and care coordination to support continuity of care and the patient’s comfort and quality of life.

What We’re Looking For

• High school diploma or GED or equivalent in training and experience
• Must have successfully completed the Certified Nursing Assistant program.
• Must be certified as a nursing assistant in the State of GA.
• At least one (1) year of experience as a Certified Nursing Assistant in a hospital, nursing home, or home health agency. Hospice experience is a plus.
• Initiative-taking, competitive, purposeful, and able to work independently.
• Compassion, empathy, and emotional intelligence when collaborating with patients, families, caregivers, and healthcare partners.
• Proficiency with EMR systems, Microsoft Office, email, and other technology
• Valid driver’s license, reliable transportation, and current auto insurance required.

Schedule: Full-Time |
Home Base: Fayetteville, GA
Service Area: Within a maximum 50-mile radius of Fayetteville, GA Salary: $18.00- $21.00/ Hour
