Description:The Primary Care LPN will coordinate patient flow from registration through discharge.The role aims to minimize wait times and ensure efficient care delivery.The LPN will perform nursing assessments and document vital signs and other required patient data.Quality metrics will be completed in the electronic medical record in accordance with established standards.The LPN will prepare patients for office visits by obtaining relevant provider notes, laboratory results, and diagnostic studies.Assistance will be provided to providers during office-based procedures, examinations, and patient care.Office visits, surgical procedures, and diagnostic studies will be scheduled, including obtaining required prior authorizations.The LPN will assess patient understanding of health conditions and care needs while providing appropriate education and guidance.Management of patient phone calls is expected, including the communication of clinical concerns to providers.Timely follow-up for urgent and non-urgent issues is necessary.Prescription refill requests will be coordinated, with laboratory and diagnostic results monitored.Urgent findings or patient concerns must be escalated to providers promptly.The LPN will perform pulmonary function testing, EKGs, phlebotomy, vaccine administration, and other clinical testing and treatments as required.Conducting CLIA-waived testing in accordance with laboratory protocols, including quality control and documentation requirements, is essential.Monitoring and maintaining office and medical supply inventory is part of the role.The LPN must recognize and respond to medical emergencies according to established protocols.
