Care Management RN: Elevate Patient Transitions

The Wexner Medical Center in Columbus, OH is seeking a Care Manager (nurse case manager) to coordinate patient care from pre-admission through post-discharge. You will work with physicians, nurses and other staff to ensure appropriate care levels and efficient resource use. Required qualifications include RN licensure and a bachelor’s degree; two years of RN experience are required, with a master’s degree preferred.
