Physical Therapy Assistant Home Health

Job Summary:

Freedom at Home, a Combat and Service Disabled Veteran owned and operated Small Business, is seeking a compassionate and dedicated Physical Therapy Assistant (PTA) to join our team of highly valued clinicians. As a PTA Home Health, you will provide skilled physical therapy services under the direction and supervision of a licensed Physical Therapist to patients in the comfort of their own homes.

Responsibilities:

• Provide skilled physical therapy services under the direction and supervision of a licensed Physical Therapist

• Carry out individualized treatment plans designed to improve patients' mobility, strength, balance, function, and overall quality of life

• Assist patients with therapeutic exercises, gait training, transfer training, balance activities, and use of assistive devices

• Monitor patient progress and communicate changes to the supervising Physical Therapist

• Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques

• Complete accurate, timely clinical documentation

• Participate in case conferences, team meetings, and quality improvement initiatives

• Collaborate with physicians, nurses, therapists, and other healthcare professionals to provide coordinated patient care

• Promote safe, compassionate, high-quality care that helps patients remain independent in their homes

Qualifications:

• Graduate of an accredited Physical Therapist Assistant (PTA) program

• Current Physical Therapist Assistant license in the state of practice

• Current CPR certification

• Minimum of two years of PTA experience preferred

• Home Health experience is a plus but not required

• Valid driver's license, reliable transportation, and auto insurance
