Integrated Behavioral Health Provider (LCSW or LPC required)Position Type: Full Time Job Shift: Day Education Level: Graduate Degree Category: Health CareDescriptionPrimary Duties:Work within a transdisciplinary primary care team, provide integrated behavioral health care and advance care management for patients with chronic conditions.Support and closely coordinate mental health care with the patient's medical provider; screen, assess and offer focused treatment within the medical setting for patients with common mental health disorders.Deliver time-limited, target-based treatments such as brief cognitive behavioral therapy and solution-focused therapy. Common interventions include behavioral activation, problem-solving therapy, stimulus control therapy, sleep hygiene, acceptance-based strategies, psychoeducation, relaxation training, motivational interviewing, and parent training.Advanced care management tasks include pre-visit planning, patient education, registry monitoring and patient self-management training.Provide care coordination service to address short- and long-term medical and behavioral health treatment goals.Communicate with physician and care team members about relevant patient needs or changes.Identify cost-effective protocols and paths for improving population health and participate in developing guidelines for care.Promote health equity by recognizing the close link between the elimination of disparities and health outcomes.Other Duties:Other duties as assigned by supervisorPhysical and Mental Requirements:Visual and auditory accuracyShift length - 8 hoursIndoor settingContinuous use of computer and calculatorLong periods of sittingFrequent use of telephoneContinuous repetitive grasping and manipulation of both handsContinuous conversational communicationOccasional reaching, walking, squatting, bending, kneeling, twisting and climbingOccasionally carrying, lifting, pushing and pulling of up to 25 lbs.Occasionally working in confined, noisy, dusty areasFrequent use of personal carUnderstand/carry out simple/detailed, oral/written instructionsMemorize and retain instructionsRead and interpret detailed specificationsMental competencyTobacco non-userPhysical accommodations will be implemented where necessary and feasibleQualificationsThe mission of Waco Family Medicine (WFM) is to improve the wellness of the underserved and vulnerable residents of the Heart of Texas by providing effective access to high quality comprehensive primary healthcare. Almost 80% of our patients live at or below the Federal Poverty Guideline and represent a diverse range of cultural backgrounds. We provide care at every stage of life from seventeen sites across two counties.Reports To: Director of Integrated Behavioral HealthHours: M-F 8:00 AM-5:00 PMSalary: $69,000–$82,400 (includes incentive pay and professional expense allowances)*Potential for Texas Loan Repayment for Mental Health Professionals up to $80,000Education & Experience: Master's degree in social work, counseling, or related field. LCSW or LPC REQUIRED.Skills:Knowledge of health care conditions and needs of patients; specific training, knowledge and interest in perinatal and maternal mental health preferred.Basic understanding of mental disorders and principles of psychotherapy.Ability to navigate health care systems. General knowledge of chronic health care conditions and needs.Ability to communicate across professional disciplines.Ability to speak Spanish is valued.Candidates with experience serving diverse or underrepresented populations is preferred. Prior demonstration of knowledge or contributions to racial justice efforts is considered a minimum qualification.
