New England Village Job Title: LPN/RN (Per Diem) Location: Pembroke, MA 02359 Salary: LPN $38.00 per hour and RN $45.00 per hour Summary of Position: The Per Diem RN/LPN plays a vital role in supporting the health, safety, and well-being of individuals across New England Village's residential and day services programs. Working collaboratively with interdisciplinary team members, program leadership, families, and healthcare providers, the Per Diem RN/LPN provides nursing assessment, care coordination, training, and clinical support that promotes quality health outcomes and person-centered supports. This position provides flexible nursing support across New England Village programs as needed and helps ensure continuity of healthcare services while fostering each individual's dignity, independence, choice, and overall quality of life. Coordination of Services: Coordination of Services: Health Assessments: Complete nursing assessments, including wellness and illness evaluations following urgent care visits, hospital visits, falls, or other health-related events, ensuring appropriate documentation, communication, and follow-up. Documentation: Complete timely and accurate documentation of assessments, communications, and follow-up needs within the assigned program or residence to support continuity and transparency in healthcare delivery. Maintain Accurate Medical Records: Assist with reviewing and updating medical and medication records, including healthcare provider orders, medication lists, immunization records, healthcare provider visits, rehabilitation records, and other healthcare-related documentation. Medical Appointments: Accompany individuals to physician appointments as needed, supporting communication and advocacy for optimal health outcomes. Advocate Through Communication: Work closely with healthcare providers, including visiting nurses, laboratories, hospice care, pharmacies, and physician offices, to support individualized healthcare plans and continuity of care. Collaborate on Treatment Plans: Monitor and support medical treatment plans while in the residence and/or program, working collaboratively with healthcare professionals, support staff, and the individual to ensure plans remain responsive and person-centered. Support Nutritional Needs: Provide dietary support with preparation of specialized diets and eating. Monitor nutrition plan compliance and assess changing needs. Facilitate Transitions of Care: Assist with new admissions and with coordinating admissions and discharges from healthcare facilities, including hospitals and rehabilitation centers, ensuring smooth transitions and continuity of care. Medication Administration: Administer treatments and medications, following the Department of Public Health's (DPH) Medication Administration Program (MAP) policies and prescriber orders. Healthcare Protocol Review: Implement and review healthcare treatments, prescriber orders, and protocols for accuracy, ensuring compliance with funder regulations and ease of use and understanding for program staff. Safety, Infection Control, and Privacy: Execute duties to reflect reasonable safety, infection control standards, and privacy standards. Universal/standard precautions must be utilized and training obtained in areas that constitute risk. Interdisciplinary Communication: Ensure prompt communication to the interdisciplinary team regarding healthcare concerns, changes in condition, follow-up needs, and recommendations. Training: Training: Provide healthcare training as needed, such as vital signs, infection control, nursing assessments, MAP certifications and recertification, and other health-related topics. Coordinate MAP trainings following DPH's assigned curriculum (RN only). Provide program-specific medical education covering diagnosis, protocols, safe care, ancillary practices, and other health-related topics in collaboration with leadership. Maintain MAP Trainer Certification (RNs only). Professionalism: Professionalism: Foster a positive, respectful work environment by promoting teamwork, cooperation, and embracing diversity. Perform other similar duties as responsibilities necessitate or as requested by supervisor. Knowledge and Experience Required: Knowledge and Experience Required: Massachusetts Licensed Registered Nurse or Licensed Practical Nurse. Valid license as an RN / LPN in the Massachusetts Board of Registration in Nursing. Licensure must remain active and in good standing. Three (3) years of nursing experience as an RN or LPN preferred. Must possess a valid driver's license and reliable transportation. Requires the use of judgment in the modification or adaptation of procedures to meet unusual situations. Works with confidential information, including individual medical records and health-related documentation. Proficient computer skills including Word, Excel, and other related Microsoft Office applications. Competent and comfortable learning new systems, programs, and applications.
