Physical Therapy Assistant (PTA) - LTAC

Physical Therapist Assistant Home Health
Schedule: 8-hour day shifts only
Assignment Details: May see up to 6 patients per day. Driving radius may be up to approximately 60 miles.
Requirements: 1 year of skilled home health experience. Certification: BCLS. Familiarity with OASIS preferred.
Roles & Responsibilities: Provide physical therapy services to patients in their homes. Develop and implement individualized treatment plans based on patient needs. Evaluate patient progress and modify treatment plans as necessary. Collaborate with healthcare team members to ensure comprehensive patient care. Educate patients and families on treatment options and home exercises. Maintain accurate and timely documentation of patient encounters.
Compliance / Notes: Business casual attire preferred; scrubs allowed, no jeans. Free parking available. Reliable personal transportation required. Must comply with COVID vaccination requirements.
