Clinical Coordinator Step down Unit RN

TriCities Hospital, part of the HCA Virginia Health System, is seeking a Clinical Coordinator Step Down Unit RN to support our progressive care team in delivering high-quality, compassionate care to medically complex patients. In this role, you will coordinate clinical operations for the step-down unit, function as a working RN at the bedside, and serve as a clinical resource and mentor for staff. You will collaborate closely with physicians, advanced practice providers, respiratory therapists, case management, and ancillary services to ensure safe, evidence-based care, appropriate patient placement, and smooth transitions across the continuum.

As the Clinical Coordinator, you will help oversee daily patient flow, including admissions, transfers, and discharges, while monitoring acuity, staffing, and assignments to support optimal patient outcomes and staff workload balance. You will participate in and often lead huddles, contribute to care planning and interdisciplinary rounds, and support escalation of care when patients require higher-level services. You will help reinforce compliance with hospital policies, core measures, infection prevention standards, medication safety practices, and documentation requirements.

You will play a key role in onboarding and developing nurses on the step-down unit by precepting new team members, providing real-time clinical coaching, and supporting competency validation in areas such as telemetry monitoring, titration of select IV medications, post-operative and post-procedural care, and early recognition of patient deterioration. You will participate in quality improvement initiatives, collect and evaluate data related to patient outcomes and unit performance, and assist leadership in implementing evidence-based practice changes that enhance quality, safety, and patient experience.

This position requires strong clinical judgment, excellent communication skills, and the ability to remain calm and organized in a fast-paced, rapidly changing environment. Responsibilities include providing direct nursing care to a diverse adult population, including cardiac, pulmonary, neurological, post-surgical, and other step-down level patients; performing thorough assessments; interpreting cardiac rhythms; implementing provider orders; safely administering medications; and documenting in the electronic health record with accuracy and timeliness. You will coordinate patient and family education related to diagnoses, medications, procedures, and discharge plans, and work closely with case management and social work to support safe discharges and reduce readmissions.

This is a full-time role in a hospital-based, field/on-site work environment, with rotating shifts that may include days, nights, weekends, and holidays based on unit needs.
