Licensed Master Social Worker (LMSW) Overview The Licensed Master Social Worker (LMSW) will provide essential psychosocial support and care coordination in a hospital setting. Engage in a 13-week contract assignment with consistent Friday–Monday scheduling, working 40 hours per week. Collaborate with interdisciplinary teams to address patient needs and ensure safe transitions of care. Utilize expertise in discharge planning and Medicare requirements to enhance patient outcomes. Work independently in a fast-paced acute care environment, providing timely interventions. Support patients and families in navigating available resources and understanding care options. Maintain accurate documentation in electronic medical records to ensure compliance and continuity of care. Contribute to a dynamic hospital-based team, enhancing the quality of care provided. Licensed Master Social Worker (LMSW) Key Responsibilities & Duties Conduct psychosocial assessments and provide interventions for patients and families in acute care settings. Assist in discharge planning, ensuring seamless transitions to post-acute services and community resources. Identify and address barriers to discharge, collaborating with interdisciplinary teams to develop solutions. Coordinate referrals to appropriate support systems, enhancing patient access to resources. Communicate effectively with patients, families, and healthcare professionals to facilitate care coordination. Ensure compliance with hospital policies and Medicare regulations, including the MOON requirements. Provide emotional and behavioral support to patients and families during hospitalization. Maintain detailed and timely documentation to support patient care and regulatory compliance. Licensed Master Social Worker (LMSW) Job Requirements Master of Social Work degree and active LMSW license are required. Minimum of 2 years of hospital or acute care social work experience. Proficiency in discharge planning, care coordination, and Medicare requirements. Knowledge of Medicare Outpatient Observation Notice (MOON) regulations. Strong communication, organizational, and documentation skills. Ability to work independently and effectively within a multidisciplinary team. Comfortable in a fast-paced hospital environment, addressing diverse patient needs.
