Home Health Physical Therapist (PT) Location: Lufkin, Texas and surrounding service area (60-mile radius) Schedule: 8 hours per day, 5 days per week Productivity Expectation: 30 points per week On-Call/Weekend Requirement: None Position Overview We are seeking a dedicated and compassionate Home Health Physical Therapist (PT) to provide skilled therapy services to patients throughout the Lufkin service area. This role allows clinicians to work independently while making a meaningful impact on patients' recovery, mobility, and quality of life in the comfort of their homes. Key Responsibilities Complete comprehensive physical therapy evaluations and assessments Perform and document OASIS assessments accurately and timely Provide skilled physical therapy treatment through subsequent and follow-up visits Develop individualized treatment plans based on patient goals and clinical needs Educate patients and caregivers on therapeutic exercises, mobility strategies, and safety precautions Collaborate with physicians, nurses, and interdisciplinary team members to coordinate patient care Maintain accurate, timely, and compliant documentation in accordance with agency and regulatory requirements Monitor patient progress and adjust treatment plans as appropriate Required Qualifications Current Physical Therapist (PT) license in the State of Texas Minimum of 1 year of home health experience Experience completing OASIS assessments Proficiency with Homecare Homebase (HCHB) Strong assessment, communication, and documentation skills Valid driver's license and reliable transportation By providing your phone number, you consent to: (1) receive automated text messages and calls from the Judge Group, Inc. and its affiliates (collectively "Judge") to such phone number regarding job opportunities, your job application, and for other related purposes.
