ESSENTIAL KEY JOB RESPONSIBILITIES Assessment: Conduct admission and ongoing assessment of patients' condition. Provide nursing care to meet the patient’s physical, psychological, and spiritual needs. Notify doctor of changes in assessment that require attention. Planning: Design, direct, evaluate, and document the plan of care, patient/family teaching, and the transition of care plan to include a safe appropriate level of care discharge. Work with other clinical disciplines to meet specialized patient care and discharge needs. Formulate a plan of care and daily goals that take into consideration the individualized needs of the patient. Implementation: Prepare patients physically, psychologically, and spiritually for treatments, surgeries, and diagnostic studies to help reduce patient anxiety. Know patients' conditions and report significant changes to Charge Nurse/Team Leader, and/or physician. Implement patient safety initiative and protocols including medication safety practices, physician orders, consult notifications. Evaluation: Reassess patients at appropriate intervals following interventions. Notify physician as necessary to modify interventions. Adjust nursing care to meet patient's emerging and changing needs. Report Skills: Give and receive report using Bedside Shift Report including family participation. Provide an accurate and comprehensive report to the oncoming shift to help establish continuity of patient care. Job Requirements Bachelor of Science in Nursing At least 1 year of RN experience at hire Demonstrated competence in Registered Nurse I performance expectations or meets experience requirements at hire Registered Nurse: Texas licensure at hire Basic Life Support – CPR within 14 days Advanced Cardiac Life Support within 6 months Where You'll Work St. Luke’s Health – The Woodlands Hospital is a 245‑bed facility serving North Harris and Montgomery counties.
